Health IT startup Axial Exchange is getting some national recognition for software that improves how patient information is shared among healthcare providers.

The Raleigh, North Carolina company’s software came out as the top technology in a health IT innovation challenge put on by the Office of the National Coordinator for Health IT, or ONC, a part of the Department of Health and Human Services. ONC, which is charged with coordinating health IT efforts to exchange health information, challenged software developers around the country to come up with applications that coordinate the exchange of healthcare information as patients move from one health setting to another, such as going from a hospital to a nursing home.

Axial’s submission, called the Care Transition Suite, coordinates patient information across an array of devices that include tablets, smartphones and laptop computers. The web-based software also offers an interactive care plan for patients to follow after discharge.

ONC launched the challenge in September in conjunction with HHS’ Partnership for Patients initiative. The challenge drew 40 submissions, each application judged on its ability to handle inpatient data, as well as its design and ease of use. For being judged the top technology, Axial wins $25,000 and the chance to demonstrate the technology at the CMS QualityNet Conference. Runners up were iBlueButton, an application from San Diego company Humetrix, and VoIDSPAN, from Palo Alto, California company Flexis.

Axial was founded by CEO Joanne Rohde, a former executive of Raleigh open source software company Red Hat (NYSE:RHT). Axial earlier this year completed a $1.5 million first round of financing for continued development and expanded sales and marketing efforts.
